Cancellation Policy: The provider understands and is responsible for a fee if (1) missing a scheduled tutoring session or (2) canceling a session by providing less than a two-hour notice. 

Outstanding Balance Policy: Account balance must be paid-in-full before attending additional scheduled sessions.

Individualized Report Policy: I, the provider, understand that the individualized student report can only be provided to students that complete the assigned pretest.

Outstanding Balance and Cancellation Policy: The account balance must be paid-in-full before attending the three-hour math test prep workshop. The provider is responsible for a $30 fee if a cancellation or no-show occurs.
